Try reloading the theme.

Customise > Load Custom UI Theme
All the standard themes should be in <installdir>\UI

Max 4.2 through 2013.
XP-64 (SP2)
NVidia 9800GTX-512 (Driver 266.58).
Core 2 Quad Q6600 2.4GHz, 8Gb Ram, DX9.0c.

Assuming your problem is exactly the same as shown in the image linked above…

Delete (or rename) that file - Max will create a default one when you next load it.

No.

Delete only only the one mentioned in the error message (to begin with). There are (possibly) many files of the form “maxstartUI.*” in several directories - locate the specific instance of that file, namely

C:\Documents and Settings\<username>\Local Settings\Application Data\Autodesk\3dsmax\<version>\enu\UI\MaxstartUI.mnu

If you then get a different (but similar) error, repeat for that file. I would suggest you rename them rather than delete just in case Max doesn’t recreate all of them - I tested the MaxstartUI.mnu but not the others.

[edit] Ahh - I see you found the right one while I was typing (slowly) ;-) [/edit]
